A. Field of Interest/Mission Statement: Last update: relative position in IEEE - overlaps, collaborations, memberships in councils and committees; position in overall technical world, mix of theoretical, practical, manufacturing.

B. Finances: Status and 5-year history, sources of income/expenses.

C. Governance: Current copy of Constitution and Bylaws; # of admin meetings per year; # of elected members, term(s), how elected, demographics; same for appointed officers.

D. Technical Activities: Description, activities.

E. Chapters: #s, locations, special programs, financial support.

F. Membership: Five -year trends and demographics; what do members get for $23; recruitment efforts; internal (IEEE) and external competition, surveys, doublet matrix analysis.

G. Meetings: For each major event: 5-year history of attendance, venues, # of papers, surplus; Submitted paper acceptance ratio, Location of authors.

H. Publications: This is also our response to the requested information and recommended format for the TAB Periodicals Review Committee.

I. Globalization Efforts: Active member and general membership demographics, Sister Societies.

J. Competition/Cooperation: US/Non-US, internal/external to IEEE, profit/not-for profit publications, meetings.

K. Standards: Number, Activity level.

L. Education: Short courses, Distinguished lecture program, Continuing education, Books/IEEE Press.)

M. Long Term Goals/New Directions: Long term planning process, new product development process, goals development and plan to accomplish, vision of Society in 5 years.

N. Interaction with TAB: Society's role in TAB, relationship with TAB, how can TAB be of more benefit.

O. Summary: Successes, opportunities, weaknesses, other comments.
